At the jobsite, employees operating MAXCN 55 nail guns while performing decking activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

At the jobsite, employees performing decking activities on the roof were not provided fall protection and exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.   .

The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
