29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about 03/05/2021, at the wood laminating automated production line, employees performed maintenance on equipment with the following hazardous energy sources: hydraulic energy, pneumatic energy, high temperature thermal energy, and electrical energy. A lockout tagout program was not established.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about 03/05/2021, at the wood laminating automated production line, employees performed maintenance on equipment with the following hazardous energy sources: hydraulic energy, pneumatic energy, high temperature thermal energy, and electrical energy. The production line machinery was shut down prior to maintenance via only a computer system rather than de-energizing the hazardous energy at its energy source.
