29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At jobsite located at 2755 N. Bracken Wichita KS, employees engaged in residential framing activities were exposed to fall hazards of approximately 12 feet from the working surfaces to the ground in that fall protection was not utilized.

29 CFR  1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  At jobsite located at 2755 N. Bracken Wichita KS, employees engaged in framing activities, the employer did not ensure that appropriate permission from the manufacturer was obtained by the user prior to using an attachment on the Terex Telehandler, and did not ensure that markings for the capacity, operation and maintenance were changed accordingly.
