On or before January 12, 2021 during the course of a new construction of an apartment complex with parking garage, located in the north east area of P2 debris were not being kept cleared from work areas adjacent to an approximate 4 foot x 4 foot unsecured covered mechanical duct work floor hole.

29 CFR 1926.502(a)(2):Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection. On or before January 12, 2021 employees were exposed to fall potentials in excess of 10 feet during the course of a new construction apartment complex parking garage, located in the north east area of P2 where employees were working adjacent to an approximate 4 foot x 4 foot unsecured covered mechanical duct work floor hole without fall protection systems which comply with Subpart M being provided or maintained before employees began that work which necessitated such protection.
