29 CFR 1910.333(a):Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: Hubbell Test Bench: On, or about, February 25, 2021, the employer had not developed and implemented safe work practices to protect employees from shock hazards while operating the marine cable test station.

29 CFR 1910.333(c)(10): An electrical safety interlock designed to protect employees from electrical hazards was defeated for purposes other than temporarily working on the equipment: Hubbell Test Bench: On, or about, February 25, 2021, the Keyence Model SZ-04M safety laser scanner that was mounted on the test bench had been adjusted to minimize the sensing field while performing quality testing on marine cable assemblies. The sensing field was too small to effectively detect an employee in the test area which bypassed the intent of the scanner to deactivate the test unit if any part of an employee's body entered the test area.
