29 CFR  1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  a) On or about March 9, 2021, between the two buildings, employees working in close proximity to power lines with an unknown voltage were not provided with electrical protective equipment including, but not limited to rubber insulating blankets, rubber insulating matting, rubber insulating covers, rubber insulating line hose, rubber insulating gloves, and rubber insulating sleeves.

29 CFR  1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a) On or about March 9, 2021, between the two buildings, employees worked in close proximity to a power line with an unknown voltage without first guarding the circuits by insulation or contacting power source for de-energizing.

29 CFR  1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit:  a) On or about March 9, 2021, between the two buildings, signs were not posted identifying overhead power lines that were in close proximity to the building. In addition, the employer did not warn employees about the hazards or the proper protective measures to be taken when they worked in close proximity to the overhead power lines.

29 CFR  1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):  a) On or about March 9, 2021, between the two buildings, employees worked from scaffolding that was erected less than 3 feet from an insulated power line with an unknown voltage without first contacting the utility company or installing protective coverings to prevent accidental contact with the lines.
