29 CFR 1926.451(e)(2): Portable, hook-on, and attachable ladders were not used to access/egress scaffold systems (Additional requirements for the proper construction and use of portable ladders are contained in subpart X of this part -- Stairways and Ladders): On or about March 8, 2021, at a commercial construction site, masonry employees were applying foam from a (3) tier tubular welded frame scaffold from the 3rd level of the scaffold without a ladder. The employees were exposed to a fall hazard of approximately 18 feet to the ground below.

The employees were exposed to a fall hazard of approximately 18 feet to the ground below.
