29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  a) On or about March 9, 2021 the Job Safety Observation Form (JSOP), Work Activity: Finishing: Top Center, Bottom Center, and Carriage Belt Changing Procedures used in conjunction with the machine lockout procedure to perform the servicing task of belt changes on the straight line gluers did not clearly and specifically outline 1) the technique(s) to ensure Team Member 1's hands would be kept away from nip points while the belt was inched into the machine and; 2) did not provide Team Member 1 control over the movement of the belt material being inched into the machine.   b) On or about March 30, 2021 the lockout / tagout procedure for M40 Bowl Formers did not clearly and specifically outline the techniques to be utilized by maintenance employees to safety release stored energy from the machine(s) when the holding brake locked up and needed to be released.
