Location: Jobsite An employee was exposed to a fall of approximately 30 feet where the scaffolding being worked on was not fully decked.

29 CFR 1926.451(c)(1)(ii):Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer's. Location: Jobsite Employees were working from a 5 tier scaffolding that was not adequately secured to the building to prevent collapse. A rope was tied around the building's chimney and piece of tie wire was used to secure the rope to the scaffolding.
