29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: a) At Southwest corner (side) of house: On or about March 9, 2021 and at times prior the employer exposed his employees to fall hazards in that he allowed his employees to work on a 4:12 pitch roof without fall protection exposing his employees to falls of 18.5 feet to the ground below.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) a) At Southwest corner (side) of house: On or about March 9, 2021 and at times prior the employer exposed his employees to fall hazards in that employees were allowed to access a roof that was approximately 18.5 feet above the ground with a ladder that was set up where the ladders side rails did not extend at least 3 feet above the upper landing surface.
