29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Job site, 1005 Airbrake Avenue, Wilmerding, PA , on or about March 8, 2021, additions were made to the Crown and Nissan forklifts, that were not rated by a PE nor was manufacturer's prior written approval given. In doing so employees were exposed to crushed by and struck by hazards while using the forklift to move 55 gallon drums.
