29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a) D.B.E. MANUFACTURING AND SUPPLY, LLC at 431 5th Street, Greeley, CO: On and preceding March 9, 2021, the employer did not ensure that an employee was protected from falling, in that an employee performing maintenance upon a furnace stood upon a platform in the ceiling space above the office area.  The platform was not provided with a guardrail system nor did the employer ensure that the employee utilized a personal fall arrest system. This condition exposed the employee to a fall hazard of approximately 8 feet.

MANUFACTURING AND SUPPLY, LLC at 431 5th Street, Greeley, CO: On and preceding March 9, 2021, the employer did not ensure that a metal electrical box was used in accordance with listed instruction, in that the box had missing knockouts. The box was equipped with a switch which was operated by employees in the forklift shop to power a heater. This condition exposed employees to shock and burn hazards.

MANUFACTURING AND SUPPLY, LLC at 431 5th Street, Greeley, CO: On and preceding March 9, 2021, the employer did not ensure that the load capacity was marked on a plate and securely affixed in a conspicuous place for the storage area above the storage room north of the sales office. Building materials were stored on the roof of the storage room.  Employees performed work in the storage room.  This condition potentially exposed employees to struck-by hazards.  (b) D.B.E. MANUFACTURING AND SUPPLY, LLC at 431 5th St, Greeley, CO: On and preceding March 9, 2021, the employer did not ensure that the load capacity was marked on a plate and securely affixed in a conspicuous place for the storage area above the machine room in the steel shop. Building materials were stored on the roof of the machine room.  Employees used the shear in the machine room. This condition potentially exposed employees to struck-by hazards.
