29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Or or about March 9, 2021, at the worksite located at N168 W19851 Main St., Jackson, WI, the employer did not ensure workers were using fall protection on the roof of a commercial building. Workers were installing coping material on a parapet wall and were not connected to the anchorage system, thereby exposing employees to fall hazards. The JT Rams, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1180478, citation number 1, item number 1, and was affirmed as a final order on November 30, 2016, with respect to a workplace located at 835 W. Johnson St., Fond Du Lac, WI 54935.

29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) Or or about March 9, 2021, at the worksite located at N168 W19851 Main St., Jackson, WI, the employer did not ensure fall protection equipment was inspected to ensure proper usage for the type of system used. Additionally, components of the fall protection system equipment such as, but not limited to, lanyards were frayed, thereby exposing employees to fall hazards.
