No. 10A-1970:   a) Fort Campbell - Salt Shed - Building 5160: On or about 3/09/2021 and days prior, a portable fire extinguisher mounted on an Arial lift (SN# 0300266554) had been discharged and was not charged and ready for use.

29 CFR  1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) Fort Campbell - Salt Shed - Building 5160: On or about 3/9/2021 and days prior, the flexible cord to a portable circular saw workers were using was damaged exposing workers to the hazard of electric shock from unprotected energized conductors.

29 CFR  1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification:   In the alternative:  29 CFR  1926.20(b)(3): Machinery, tool(s), material(s), or equipment which was not in compliance with the applicable requirement(s) of 29 CFR 1926 was not prohibited, and/or was not either identified as unsafe by tagging, or locking the controls to render them inoperable, or physically removed from its place of operation:  a) Fort Campbell - Salt Shed - Building 5160: On or about 3/9/2021 and days prior, the employer exposed workers to potential fall and crushing hazards, Workers were working from an JLG Model 450AJ articulating boom aerial lift (SN# 03002700348) and the emergency stop safety cable had been modified/rigged after being damaged bypassing the safety mechanism and  to enabling the machine to fully operate.
