29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Building Interior:  Employees were exposed to head injuries while framing  multi family residential home. The employees  were not protected by a hard hat or protective helmet, on or about 3/10/21.  b) Building Exterior, On the ground:  An employee was exposed to head injuries while employees where framing the roof level of a multiple family  residential home. The employee  was not protected by a hard hat or protective helmet, on or about 3/10/21.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                            a)  Exterior roof level: Employees were exposed to eye/face injuries while framing with a pneumatic nail gun at the roof level, and they were not protected with any type of eye/face protection, on or about 3/10/21

29 CFR 1926.150(a)(1): The employer did not develop and maintain a fire protection program to be followed throughout all phases of construction and demolition work, and/or did not provide for the firefighting equipment as specified in this subpart:  a) RHM Construction LLC, does not ensure that employees have available firefighting equipment at the jobsite. The employer did not provide any firefighting equipment at the jobsite. Combustible products such as wood, cardboard, Gasoline fueled generators was used on site to power electric equipment such as, nail guns, extension cords and compressors to perform  framing activities. Extra gasoline to fuel the generators was stored in gas cans on the site, on or about 3/10/21.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Building Exterior - Roof: Employees were exposed to a 30 foot fall hazard while working on a  roof of a residential home without the use of personal fall arrest system or other means of fall protection in place, on or about 3/10/21.

29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a)  Roof level: Employees were exposed to a 10 foot fall hazards while framing the roof floor and they could fall through holes/floor opening  and were not protected by personal fall arrest system, cover or guardrails systems, on or about 3/10/21.
