29 CFR  1926.453(b)(2)(iv):Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  5325 #2 Lone Star Parkway, San Antonio TX: On or about December 17, 2020, and at times prior thereto, workers were exposed to fall hazards when climbing in and out of an elevated Genie S-40 aerial lift basket to perform metal decking weld operations.

29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  5325 #2 Lone Star Parkway, San Antonio TX: On or about December 17, 2020, and at times prior thereto, workers were exposed to fall hazards while working in an elevated Genie S-40 aerial lift without the use and wear of a personal fall harness.

29 CFR  1926.453(b)(2)(vi):Boom and basket load limits specified by the manufacturer were exceeded:  5325 #2 Lone Star Parkway, San Antonio, TX: On or about December 17, 2020, and at times prior thereto, workers were exposed to fall hazards as they exceeded the Genie S-40 aerial lift maximum occupancy limit while lowering themselves to the ground.
