29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about March 10, 2021, employees were exposed to a struck by hazard while performing framing activities on a residential home located at site 1592 Amber Way, North Ridgeville, Ohio 44039. The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses. A.M. Jr. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFE 1926.102(a)(1), which was contained in OSHA inspection number 1216129, citation number 1, item number 1, and was affirmed as a final order on 4/20/2017, with respect to a workplace located at 32838 Rebecca Lane, Avon Lake, Ohio 44012.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about March 10, 2021, employees were exposed to a fall of approximately 22 feet while performing framing activities on a residential home located at site 1592 Amber Way, North Ridgeville, Ohio 44039. The employees were working on the roof and standing on a window opening and the employer did not ensure that they were using fall protection. A.M. Jr. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFE 1926.501(b)(13), which was contained in OSHA inspection number 1216129, citation number 1, item number 2, and was affirmed as a final order on 4/20/2017, with respect to a workplace located at 32838 Rebecca Lane, Avon Lake, Ohio 44012.
