29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about March 2, 2021, on line 34, the employer did not ensure that the company's energy control procedures were utilized to control the electrical energy sources on the inline grinder for line 34 when employees conducted activities such as, but not limited to clearing jams exposing employees to caught in hazards with potential laceration, fracture and amputation injuries.

29 CFR 1910.147(c)(7)(i)(A): The employer did not provide training to authorized employee(s) on the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: On or about March 2, 2021, the employer did not provide training to the authorized employees which covered the specific methods and means necessary for energy isolation and control on equipment such as the inline grinders in that an employee was unjamming the line 34 grinder and did not shut down the equipment prior to performing the task.
