On March 10, 2021 an employee was fatally injured from a fall through an open stairway.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) First floor - On or about March 10, 2021 and at times prior thereto, employees were exposed to fall hazards while installing a set of pre-fabricated stairs. The employer did not provide adequate training on how to recognize and eliminate such hazards. On March 10, 2021 an employee was fatally injured from a fall through an open stairway.
