29 CFR  1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit."  (a) Powerhouse - 22 Gallery - Station Service to Bay 22 - On or about March 9, 2021 and at times prior thereto, the exit routes throughout the 22 gallery including, but not limited to: Station Service through Bay 22 were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (b) Powerhouse - 22 Gallery - Fish Units to Station Service -  On or about March 9, 2021 and at times prior thereto, the exit routes throughout the 22 gallery including, but not limited to: the Fish Units through Station Service were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (c) Powerhouse -  4th Floor - SU1 Bay(s) 2 & 3 - On or about March 9, 2021 and at times prior thereto, the exit routes throughout the 4th floor including, but not limited to: Bay(s) 2 & 3 were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (d) Powerhouse -  4th Floor - SU2 Bay(s) 6 & 7 - On or about March 9, 2021 and at times prior thereto, the exit routes throughout the 4th floor including, but not limited to: SU2 Bay(s) 6 & 7 were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (e) Powerhouse - 4th Floor - SU3 Bay(s) 10 & 11 -  On or about March 9, 2021 and at times prior thereto, the exit routes throughout the 4th floor including, but not limited to: SU3 Bay(s) 10 & 11 were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (f) Powerhouse - 4th Floor - SU4 Bay 4 -  On or about March 9, 2021 and at times prior thereto, the exit routes throughout the 4th floor including, but not limited to: SU4 Bay 4  was not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (g) Powerhouse - 4th Floor - Bay 17 -  On or about March 9, 2021 and at times prior thereto, the exit routes throughout the 4th floor including, but not limited to: Bay 17 was not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (h) Powerhouse - 5th, 6th, 7th, 8th, 9th Floor(s) - East & West stairwell exits - On or about March 9, 2021 and at times prior thereto, the exit routes throughout the 5th, 6th, 7th, 8th, 9th Floor(s) including, but not limited to:  the East & West stairwell exits were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (i) Powerhouse - 5th, 6th, 7th, 8th, 9th Floor(s) - Bay 17 stairwell exit -  On or about March 9, 2021 and at times prior thereto, the exit routes throughout 5th, 6th, 7th, 8th, 9th Floor(s) including, but not limited to: the Bay 17 stairwell exit was not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (j) Powerhouse - 5th, 6th, 7th, 8th, 9th Floor(s) - Bays 3 & 4 -  On or about March 9, 2021 and at times prior thereto, the exit routes throughout the  5th, 6th, 7th, 8th, 9th Floor(s) including, but not limited to: Bays 3 & 4 were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (k) Spillway - On or about March 9, 2021 and at times prior thereto, the exit routes throughout the Spillway were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (l) Non - Overflows - On or about March 9, 2021 and at times prior thereto, the exit routes throughout the Non - Overflows were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (m) Fishway Galleries - On or about March 9, 2021 and at times prior thereto, the exit routes throughout the  Fishway Galleries were not clearly visible, this exposed employees to fire and smoke inhalation hazards.  (n) Navlock Galleries - On or about March 9, 2021 and at times prior thereto, the exit routes throughout the Navlock Galleries were not clearly visible, this exposed employees to fire and smoke inhalation hazards.   The US Army Corps of Engineers, The Dalles Dam was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(b)(2), which was contained in OSHA inspection number 1453675, citation number 1, item number 1/a and was affirmed as a final order on 5/26/2020, with respect to a workplace located at The Dalles Dam.

29 CFR  1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent.  a) Powerhouse - 22 Gallery - Station Service to Bay 22 - On or about March 9, 2021 and at times prior thereto, the exit route direction of travel was not immediately apparent and did not clearly indicate the travel direction to the nearest exit(s) throughout the 22 gallery including, but not limited to: Station Service through Bay 22 , this exposed employees to fire and smoke inhalation hazards. (m) Fishway Galleries - On or about March 9, 2021 and at times prior thereto,  the exit route direction of travel was not immediately apparent and did not clearly indicate the travel direction to the nearest exit(s)  throughout the  Fishway Galleries, this exposed employees to fire and smoke inhalation hazards.  (n) Navlock Galleries - On or about March 9, 2021 and at times prior thereto,  the exit route direction of travel was not immediately apparent and did not clearly indicate the travel direction to the nearest exit(s) throughout the Navlock Galleries, this exposed employees to fire and smoke inhalation hazards.   The US Army Corps of Engineers, The Dalles Dam was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(b)(2), which was contained in OSHA inspection number 1453675, citation number 1, item number 1/b and was affirmed as a final order on 5/26/2020, with respect to a workplace located at The Dalles Dam.
