29 CFR  1910.305(a)(2)(i): Temporary electrical power and lighting installation(s) 600 volts, nominal, or less were used in prohibited manners:  (a) Welding, Metal, and Woodshop - On or about August 23, 2022 and at times prior thereto, employees were exposed to an electrical shock via the flexible extension cord that was permanently installed, with zip ties, to existing conduit. The flexible cord was energizing a welding unit in the shop.   Note:  Temporary electrical power and lighting installation(s) may only be used as specified by 1910.305(a)(2)(i)(A) through 1910.305(a)(2)(i)(C).

29 CFR 1910.159(c)(2): The employer did not properly maintain the installed automatic sprinkler system.  (a) Powerhouse - Station Service Transformer Vaults - TEMCO Fine Water Mist System - On or about August 23, 2022 and at times prior thereto, employees were exposed to fire, smoke and explosion hazard(s) in that the fire suppression system's control panel failed inspection and therefore can not be properly inspected or tested.  The US Army Corps of Engineers, The Dalles Dam, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1910.159(c)(2), which was contained in OSHA inspection number 1453675, notice number 1, item number 3a and was affirmed as a final order on 5/26/2020, with respect to a workplace located at The Dalles Dam.

29 CFR  1910.164(c)(1) The employer did not maintain all systems in an operable condition.   (a) Mechanical Shop -  On or about August 23, 2022, and at times prior thereto, employees were exposed to fire and smoke inhalation without a properly working smoke detector. The smoke detector was covered by plastic during renovation and was not removed after completion of the work.
