Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: On March 9, 2021, employees engaged in residential framing activities utilizing a circular saw at the jobsite located at 708 44th Terrace Kansas City, MO. Employees were exposed to electrical shock hazards in that proper strain relief was not provided on the Kobalt circular saw, model: K15CS-06AC serial# 004688.

On March 9, 2021 at the jobsite located at 708 44th St. Terrace Kansas City, MO 64111, employees were exposed to fall hazards. The employer failed to provide fall protection to employees working up to approximately 18' 10" while performing framing work on the third floor of a single family home in that fall protection was not provided.
