29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about March 10, 2021, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 5657 Ease Breezeway, North Ridgeville, Ohio 44039. Employees were working on the roof with tools and materials that could have fallen on lower employees and the employer did not ensure that the lower employees were wearing protective helmets.   On Top Roofing & Renovations LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a)(1), which was contained in OSHA inspection number 1236018 citation number 1,  item number 1 and was affirmed as a final order on (9/5/17), with respect to a workplace located at 35290 Detroit, Avon. Ohio 44011.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 10, 2021, employees were exposed to a fall hazard of approximately 11 feet while performing roofing activities on a residential home located at 5657 Ease Breezeway, North Ridgeville, Ohio 44039. Employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.   On Top Roofing & Renovations LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1485708 citation number 1,  item number 2 and was affirmed as a final order on (12/23/20), with respect to a workplace located at 430 Main, Huron, Ohio.   On Top Roofing & Renovations LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1364171 citation number 2,  item number 1 and was affirmed as a final order on (2/11/19), with respect to a workplace located at 3413 Rocky River Drive, Cleveland, Ohio 44102. 29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about March 10, 2021, employees were exposed to a fall hazard of approximately 11 feet while performing roofing activities on a residential home located at 5657 Ease Breezeway, North Ridgeville, Ohio 44039. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.   On Top Roofing & Renovations LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1236018 citation number 1,  item number 5 and was affirmed as a final order on (9/5/17), with respect to a workplace located at 35290 Detroit, Avon.
