29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields: On or about March 11, 2021, employees were exposed to a cave in while performing sewer installation on a commercial site located at 14520 Tare Creek, Middlefield, Ohio 44062. The trench was approximately 13 feet deep and the employer did not ensure that employees were protected from a cave in when entering and exiting the protective shield.
