29 CFR 1926.451(b)(5)(ii):The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. On or about March 11, 2021, at the 180 Main Street, South Glastonbury, CT job site: The pump jack scaffold system in use had a 22 ft long platform that extended over its end support by more than 10 ft. The scaffold was not designed or installed so that the cantilevered portion of the platform was able to support employees without tipping and the scaffold did not have a guardrails which block employee access to the cantilevered end.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. On or about March 11, 2021 at 180 Main Street, South Glastonbury, CT job site: The pump jack system that was in use, was not installed with standard guardrails around open sides of the scaffold, exposing those using the scaffold to fall hazards in excess of 12 ft.

29 CFR 1926.452(j)(3): When guardrails are used for fall protection, a workbench may be used as the toprail only if it meets all the requirements in paragraphs (g)(4)(ii), (vii), (viii), and (xiii) of 1926.451. On or about March 11, 2021, at the 180 Main Street, South Glastonbury, CT job site: The work bench that was being used as a top rail on the pump jack scaffold system did not meet the requirements to be used as substitute for a top guardrail. The bench was not located between 38" and 45" above the working surface on both ends and the work bench was not secured to the structure to ensure that it could withstand a force of 200 lbs in any direction without failure in the event that someone fell against it.
