29 CFR  1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  a.  1881 Athens Street  (129 South), Jefferson, GA (Throughout Facility) -  Employees were exposed to entrapment hazard.  There were designated, marked, and identified emergency exit doors located throughout the facility which were hidden by the large stationary machineries and overhead pipes, were not in the line of sight. There were no directional exit markings indicating the way to travel in the event of an emergency, on or about 03/12/2021.

29 CFR  1910.145(c)(2)(i): Caution signs shall be used only to warn against potential hazards or to caution against unsafe practices:  a.  1881 Athens Street (129 South), Jefferson, GA (Throughout the facility) - Employees were exposed to pinch point hazard.  Employees are working on various types of machines which housed moving parts.   The employees are exposed to pinch point  and rotating part areas on the stationary machines.  There were no caution signs/placards posted on the machines alerting employees of pinch point and/or rotating parts area on the machines, on or about 03/12/2021.

The employer failed to re-trained affected and authorized employees on lockout/tagout, after an unsafe act resulting in injuries occurred on the multi-energy source machines, on or about 03/12/2021.
