29 CFR 1926.100(a): The employer did not ensure employees were protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. On or about March 11, 2021, an employee engaged in steel erection was not wearing a protective helmet and was exposed to struck-by hazards from falling objects overhead.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: On or about March 11, 2021, employees were exposed to a fall hazard in the following instances: 1) An employee was observed working approximately 14-feet above the ground standing on the mid-guardrail of a scissor lift. 2) Two employees were observed working approximately 10-feet above the ground standing on a metal beam that was approximately 6-inches in width.
