29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): (a) Colorado Structures, Inc., dba CSI Construction, at 1100 S Public Road, Lafayette, CO 80026: On and preceding February 6, 2021, CSI Construction did not certify that a forklift operator was trained and evaluated and found competent to operate the forklift. The employer directed the work of a temporary employee. The employee operated a rough terrain extendable boom forklift. The employee, while operating the forklift, was involved in an accident. This condition exposed employees to fall hazards and struck-by hazards.
