29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Employee(s) worked disposing of debris in the trash dumpster and fetching roofing material(s) at street level out front of 248 Port Richmond Avenue in Staten Island, NY 10302. Employees were exposed to hazard of falling tools or other object(s) from roofing work directly overhead.     Condition(s) were noted on or about 3/12/2021.

29 CFR  1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  a) Debris from removal of old flat asphalt roof was tossed from the front roof edge of two-story residential property at 248 Port Richmond Ave in Staten Island NY 10302.  An enclosed chute was not used to prevent struck-by hazard of falling debris.  Condition(s) were noted on or about 3/12/2021.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) A Werner extension ladder was leaned against the front facade of a two-story residential building at 248 Port Richmond Avenue in Staten Island NY 10302, and used by employee(s) for roof access / egress during roof replacement work. The ladder side rails did not extend a minimum of 3 feet above the roof landing, exposing employee(s) to 21 feet fall hazard.  Condition(s) were noted on or about 3/12/2021.
