29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:  a) Jobsite, east side of roof:  On March 8, 2021 and at times prior thereto the fall protection system that was in use was not relocated as needed.  As the work progressed on a 1 in 12 pitched roof, employees removed the anchor points to allow placement of plastic or staging of other roofing materials on the roof deck.  This exposed employees to a 36 foot fall from the roofs edge to the muddy ground below.    b) Jobsite, west side lower roof: On March 8, 2021 and at times prior thereto employees performing roofing work on the flat, lower roof were not protected by a fall protection system appropriate for this area. The employees were exposed to an 8 foot fall hazard to a scaffold system installed below.     Note) Certification of corrective action is required for this item.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:   a) Mezzanine area on roof: On March 8, 2021 and at times prior thereto a guardrail system was not in place around a floor hole that was being used for ladder access to the mezzanine and roof deck. This exposed the employees to a 15 foot fall hazard to the floor below.   Note) Certification of corrective action is required for this item.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways with pan stairs where the treads and/or landings were not filled in with concrete or other material, and the stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan:  a) Jobsite: On March 8, 2021 and at times prior thereto employees were exposed to a trip and fall hazard while using a permanent metal stairway for access to/from the second floor level.  Wooden material that was used in the stair pans left them mostly empty, and was not flush with the top edge of each pan, creating the trip hazard.    Note) Certification of corrective action is required for this item.
