(a) On or about March 15, 2021, employees were installing windows and utilizing pneumatic nail guns without the use of protective eyewear. Employees were exposed to eye and face injuries.

Each pump jack bracket shall have two positive gripping mechanisms to prevent any failure or slippage:   (a) On or about March 15, 2021 employees were installing windows from a Werner pump jack scaffold and the pump jack brackets and bracing was made from wood materials at the jobsite. Employees were exposed to scaffold collapse hazards.
