(a) In Building 140, Production Center 5, at the a Russel Model RS-12-1/2 PC- 5 saw and Rexroth Hydraulic Unit Model #M-010-195-00-N, on or about 3/2/2021, an employee was exposed to an unguarded ingoing nip point hazard at the hold down roller assembly. Direct exposure to the hazard resulted in a fracture and multiple lacerations to the employee's left hand.
