29 CFR  1926.451(g)(3)(i): When vertical lifelines were used, they were not fastened to a fixed safe point of anchorage, were not independent of the scaffold, and were not protected from sharp edges and abrasion:  a)  Casey Industrial, Inc. 21404 I-70, East Side Eisenhower Tunnel, Dillon, CO  80435: On or about April 9, 2021 and preceding, the employer did not ensure that when vertical lifelines were used, they were fastened to a fixed safe point of anchorage, were independent of the scaffold, and were protected from sharp edges and abrasion. Employees installed conduit while standing upon an incompletely assembled tubular metal supported scaffold.  The employer required the employees to use a personal fall arrest system, using the cross members of the scaffolding to act as an anchorage. This condition exposed employees to an approximate  20 foot  fall hazard.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment was not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached:  a)  Casey Industrial, Inc. 21404 I-70, East Side Eisenhower Tunnel, Dillon, CO  80435:  On or about April 9, 2021 and preceding, the employer did not ensure that anchorages used for attachment of personal fall arrest equipment to cross members of scaffolding were independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.  Employees installed conduit while standing upon an incompletely assembled tubular metal supported scaffold. The employer required the employees to use a personal fall arrest system, using the cross members of the scaffolding to act as an anchorage.  This condition exposed employees to an approximate 20 foot fall hazard.

29 CFR  1926.1101(k)(8)(vii):  When the employer identified previously installed PACM and/or ACM, labels or signs were not affixed or posted so that employees would be notified of what materials contain PACM and/or ACM:   a)  Casey Industrial, Inc. 21404 I-70, East Side Eisenhower Tunnel, Dillon, CO  80435:  On or about April 9, 2021 and preceding, the employer identified previously installed PACM and/or ACM, and did not label or affix or post signs so that employees would be notified of what materials contain PACM and/or ACM. Employees threaded conduit in the MCC area directly adjacent to covered fireproofing (foam and foil layer over ACM fireproofing).   The fireproofing contained  3% chrysotile and 2-3% amosite asbestos. This condition exposed employees to an asbestos hazard.

29 CFR  1926.1101(k)(9)(i): The employer did not institute, at no cost to employees, a training program for all employees who were likely to be exposed in excess of the PEL and for all employees who performed Class I through IV asbestos operations:  a)  Casey Industrial, Inc. 21404 I-70, East Side Eisenhower Tunnel, Dillon, CO  80435:  On or about April 9, 2021 and preceding, the employer did not provide training for employees who performed Class III asbestos work.  Employees threaded conduit in the MCC area in close proximity to covered fireproofing (foam and foil layer over ACM fireproofing).  The fireproofing contained  3% chrysotile and 2-3% amosite asbestos.  The ACM was likely to be disturbed.  This condition exposed employees to an asbestos hazard.
