On or about March 16, 2021, the employer did not conduct adequate frequent and regular inspections of the job site, materials, or equipment being used thus exposing employees to fall hazards such as but not limited to 11 foot 7 inches, 13 foot 7 inches, and 15 foot 9 inches during roofing activities on a newly constructed commercial building roof.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a.) On or about March 16, 2021, employees were exposed to an 11 foot 7 inch and a 13-foot 7-inch fall hazard while performing roofing work on a newly constructed commercial rooftop and not using any means of conventional fall protection.

Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. a.) On or about March, 16, 2021, employees were exposed to a 15 foot 9 inch fall hazard from five second story windows that had unprotected openings during construction activities taking place at a new veterinary hospital building addition.
