29 CFR 1910.212(a)(3)(i): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (A) The point of operation was not guarded as required. WORKSHOP: The point of operation of the rotating drill spindle of the Clausing Miller vertical turret milling machine was unguarded. On/or about March 12, 2021, an employee operating the unguarded rotating spindle at the point of operation on the Clausing Miller vertical turret milling machine, operating at approximately 220 to 360 revolutions per minute (rpm) with the N992R bronze gear pump with gloves. The gloves of the employee was pulled into the unguarded spindle resulting in a middle finger amputation on the right hand.
