29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a)Woodland Cabinetry, Inc. at 2415 East Highway 10, Sisseton, SD 57262: On or about and at times prior to January 7, 2021, the employer did not ensure employees were protected from struck-by hazards involving the Gabbiani Galaxy 115 High Speed Macchine. The rear access to the Gabbiani was missing the magnetic door interlock which allowed the operator or other employees to open the door while the machine was in operation, exposing employees to being struck-by moving parts. This condition exposed employees to struck-by hazards associated with moving parts.

29 CFR  1910.213(c)(1): Circular handfed ripsaws were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  (a)Woodland Cabinetry, Inc. at 2415 East Highway 10, Sisseton, SD 57262:  On or about and at times prior to January 7, 2021, the employer did not ensure that the SawStop 10" professional Cabinet Saw in the framing area was guarded by a hood to completely enclose the portion of the saw above the table and above the material being cut.  Employees were utilizing the SawStop table saw used for Dado cuts with no guard in place.  This condition exposed employees to amputations hazards associated with the unguarded blade.  (b)Woodland Cabinetry, Inc. at 2415 East Highway 10, Sisseton, SD 57262:  On or about and at times prior to January 7, 2021, the employer did not ensure that the Dado saw in the Specialty Door Box area was guarded by a hood to completely enclose the portion of the saw above the table and above the material being cut.  Employees were utilizing the table saw for Dado cuts with no guard in place.  This condition exposed employees to amputations hazards associated with the unguarded Dado blade.

29 CFR  1910.213(m)(1):Cutting heads of wood shaper(s) were not enclosed with a cage or adjustable guard so designed so as to keep the operator's hand away from the cutting edge:  (a)Woodland Cabinetry, Inc. at 2415 East Highway 10, Sisseton, SD 57262:  On or about and at times prior to January 7, 2021, the employer did not ensure that the arch router in the door area was guarded to ensure to keep the operator's hands away form the cutting edge.  Employees were utilizing the arch router with no guard in place covering the cutting head.  This condition exposed employees to laceration hazards associated with the unguarded cutting head.  (b)Woodland Cabinetry, Inc. at 2415 East Highway 10, Sisseton, SD 57262:  On or about and at times prior to January 7, 2021, the employer did not ensure that the table router in the parts department area was guarded to ensure to keep the operator's hands away form the cutting edge.  Employees were utilizing the table router with no guard in place covering the cutting head.  This condition exposed employees to laceration hazards associated with the unguarded cutting head.  (c)Woodland Cabinetry, Inc. at 2415 East Highway 10, Sisseton, SD 57262:  On or about and at times prior to January 7, 2021, the employer did not ensure that the table router in the specialty door box area was guarded to protect the operator's hands from the cutting edge. 29 CFR  1910.213(s)(9): Suitable sizes and types of push sticks or push blocks were not provided at the work place:  (a)Woodland Cabinetry, Inc. at 2415 East Highway 10, Sisseton, SD 57262:  On or about and at times prior to January 5, 2021, the employer did not ensure that special hand tools were provided to employees while utilizing the Castle TSM-35 Pocket Jig in the framing area.  Employees were utilizing the pocket jig and placing their hands within the point of operation and under the guard that was in place for wood pieces which was approximately four inches in length.  This condition exposed employees to laceration hazards associated with the working within the point of operation of the machine.
