29 CFR  1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    On or about March 12, 2021, on the eastside of silo, employees were exposed to fall hazards while working from on an elevated wooden pallet that was not attached to the forks as they were being elevated horizontally to a work surface.

29 CFR  1926.760(a)(1):Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.     On or about March 12, 2021, on top of a concrete silo, employees were exposed to a fall of more than 15 feet when installing guardrails on a walking/working surface without being protected by a personal fall arrest system.
