29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Hotel Main Roof - An employee used a leaf blower to blow roofing debris off the roof, and employees used pneumatic nailers to install new roofing materials.  The employees did not use any eye protection, exposing them to the hazard of getting struck in the eyes by flying particles/objects, on or about March 17, 2021.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Hotel Main Roof - Employees worked on the main roof and dormers, which all had a 6 in 12 slope.  These employees worked with no protection from falling four stories to the ground below, on or about March 17, 2021.  No abatement certification or documentation required.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   (a) Hotel Main Roof - Two employees doing roofing work on the main roof, which had a 6 in 12 slope, attached their harnesses to rope lines using a single hip positioning ring rather than the dorsal D-ring, exposing the employees to the hazards of falling four stories to the ground below, on or about March 17, 2021.   No abatement certification or documentation required.
