29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are maintained in a safe condition: Riser: Employer is exposing employees to hazardous conditions where the employees are permitted to work on walking-working surface that is not maintained in a safe condition. On 5/10/2021 holes in the walking-working surface of the Ammonia Riser were measured at approximately 4 inches, in the least dimensions, exposing employees to unsafe conditions.
