29 CFR 1910.29(b)(2):  Midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high:.  Employer is failing to protect employees from fall hazards associated with inadequate guard railing systems. This was most recently documented on March 17, 2021 in the facility located at 2900 C St. Omaha, Nebraska 68107. Where the employer has employees exposed to fall hazards in the scope of their duties as a result of inadequate guard railing systems.  Instance of exposure include but are not limited to:   i.	Employees using the stairway as means of access to the U.P. area and the top landing platform guard railing is missing the mid-rail.    ii. Removed

29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  Employer is failing to protect employees from amputations hazards. This was most recently documented on March 17, 2021 at 2900 C St. Omaha, Nebraska 68107 at the entrance to the 300 tunnels. Where the employer has employees operating or working in close proximity to equipment with inadequate machine guarding.  Instances of exposure include but are not limited to:    i.	Employees crouching under the unguarded conveyer belt running  across the entrance to the 300 tunnels to access the tunnel.  ii.	Removed

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes   Employer is failing to protect employees from amputation and , laceration hazards. This was most recently documented on March 17, 2021 in the facility located at at 2900 C Street, Omaha, Nebraska 68107 in the milling area of the facility. Where the employer has employees operating, maintaining and adjusting the milling machines while exposed to inadequately guarded belts and pulleys. This was most recently documented on March 17, 2021 in the facility located at 2900 C St. Omaha, Nebraska 68107.  Where the employer has employees working in the maintenance shop where they store the  oxygen-gas cylinder approximately 5 feet from  an acetylene-gas cylinder and a wall mounted electrical box.
