Employees were not applying their own locks before entering the machine to perform maintenance. Employees were exposed to struck-by and caught-in-between hazards when entering the retort loader without applying their own lock, on or about March 16, 2021. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The retort loader was not isolated from its energy sources when being accessed by employees for maintenance. Employees were exposed to struck-by and caught-in-between hazards when entering the retort loader without isolating the machine from its energy sources. Employees were only locking out the retort loader operator access gate interlock before entering the machine, on or about March 16, 2021. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
