29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. MDO Process Warehouse: On or about March 08, 2021, and at times prior, employees were exposed to caught-in hazards, in that the employer did not ensure that control of hazardous energy/lockout-tagout (LOTO) procedures were utilized by operators performing servicing and maintenance including, but not limited to, clearing jams at the nip roller in the winder section of the MDO machine.
