29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:    (a) Scott Free Plumbing, Inc. at 1100 S. Public Road, Lafayette, CO: On 2/6/2021, the employer did not ensure that employees who performed work on a scaffold more than 10 feet above a lower level were protected from falling.  An employee was elevated approximately 20 feet while standing within a dumpster positioned upon the fork blades of a rough terrain extended boom forklift. The employee fell and sustained serious injuries. This condition exposed the employee to an approximate twenty foot fall hazard

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:   (a) Scott Free Plumbing, Inc. at 1100 S. Public Road, Lafayette, CO: On 2/6/2021, the employer did not ensure that an industrial truck was equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel. A rough terrain extended boom forklift was used to elevate an employee. The employee stood within a trash dumpster that was supported on the forks of the forklift. The trash dumpster was not secured to the lifting carriage or forks of the forklift. This condition exposed the employee to a fall hazard of approximately 20 feet.

29 CFR 1926.454(a): Employees who perform work while on a scaffold were not trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   (a) Scott Free Plumbing, Inc. at 1100 S. Public Road, Lafayette, CO: On and preceding 2/6/2021, the employer did not ensure that employees who performed work while on a scaffold were trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Employees installed concentric vents on the exterior sides of Building A on the second level. The employees performed these tasks using an aerial lift. The employer did not ensure that all employees working from the aerial lift received scaffold training. This condition exposed employees to an approximate twenty foot fall hazard.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  (a) Scott Free Plumbing, Inc. at 1100 S. Public Road, Lafayette, CO: On and preceding February 7, 2021, the employer did not report within 24 hours a work-related incident resulting in the hospitalization of an employee. The employee sustained multiple bone fractures in a fall from a height of approximately 20 feet. The employee was hospitalized as a result of his injuries.
