29 CFR  1910.95(k)(1):  The employer shall train each employee who is exposed to noise at or above an 8-hour time weighted average (TWA) of 85 decibels (dBA) in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program:  At a facility located at 1212 N. Enterprise Drive, De Pere, WI 54115; a Machine Operator employee working at the MultiCam Series 3000 CNC router was exposed to noise at 66.9% (noise dose) during a sample period of 265 minutes during one shift on March 19, 2021. 66.9% is equivalent to an 8-hour TWA sound level of 87.1 dBA.  The employer did not train the employee on the hazards of noise or how to select and properly use hearing protection.

29 CFR  1910.178(m)(2):  No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty:  At a facility located at 1212 N. Enterprise Drive, De Pere, WI 54115; employees passed under the elevated portions of forklift trucks:  a)  In Building 1, an employee walked under the elevated forks and load of a forklift while it accessed storage wracking adjacent to the CNC Router/Beam Saw area.  b)  In the Building 2, an employee walked under the elevated forks and load of the Hubtex (Machine No. 66.301) lifter while it was moving pallets of corrugated plastic sheets.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  At a facility located at 1212 N. Enterprise Drive, De Pere, WI 54115; the MultiCam Series 3000 (S# 3-208-R10970) CNC Router was not provided with machine guarding to protect the Machine Operator from the struck-by, pinch-point, caught-between and point of operation hazards presented by the moving bridge, moving carousel and rotating cutting head of the router.

29 CFR  1910.147(c)(6)(i):  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:  At a facility located at 1212 N. Enterprise Drive, De Pere, WI 54115; the employer did not conduct annual periodic inspection of energy control procedures to ensure the procedures and the requirement of this standard were being followed.  All provisions of 29 CFR 1910.147(c)(i)-(ii) shall be included in annual periodic inspections. Key elements include, but are not limited to the following:  1) Performed by a LOTO authorized employee other than the one(s) utilizing the procedure being inspected; 2)  Include review between inspector and each authorized employee of the employee's responsibilities; and 3) Certification identifying  equipment utilized, date of inspection, employees included in the inspection and person performing the inspection.  The Amerilux International LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1387202, citation number 1, item number 5a and was affirmed as a final order on September 9, 2019, with respect to a workplace located at 1212 N. Enterprise Drive, De Pere, WI 54115.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:  At a facility located at 1212 N. Enterprise Drive, De Pere, WI 54115;   a) The Hendrick Saw rotary saw blade, used for cutting corrugated polycarbonate, was inadequately guarded to prevent employees from being able to reach the exposed lower portion of the blade from below the metal mesh guard at the infeed side of the saw's table. The metal mesh guard presented an opening with the smallest dimension of approximately 6 inches with a distance of approximately 13.75 inches from the opening in the guard to the operating portion of the saw blade. Additionally, an opening in the metal mesh guard at the saw's control box presented an opening with the smallest dimension of approximately 10 inches with a distance of approximately 25.5 inches from the opening in the guard to the operating portion of the saw blade.  b) A Blade Cutter Slitter Table, used for cutting multi-walled polycarbonate, was inadequately guarded to prevent employees from being able to reach the exposed cutter blade from below the metal mesh guard at the infeed side of the cutter's table. The metal mesh guard presented an opening with the smallest dimension being approximately 1.75 inches with a distance of 9.5 inches from the opening in the guard to the slitter's blade cutting path. Additionally, the home position of the cutter's carousel was located outside the metal mesh guard and was an unguarded distance of approximately 12.5 inches from the operator's position to the exposed portion of the blade.   The Amerilux International LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1387202, citation number 1, item number 6 and was affirmed as a final order on September 19, 2019, with respect to a workplace located at 1212 N. Enterprise Drive, De Pere, WI 54115.
