29 CFR 1910.27(b)(2)(iii): The employer did not ensure that each employee who uses the rope descent system is trained in accordance with � 1910.30: (a) Maes Cleaning and Building Services LLC at 1600 20th Street, Denver, CO 80204: On and before March 12, 2021, the employer did not ensure that employees performing window cleaning operations who used the rope descent system were trained in accordance with � 1910.30. An employee washing windows of a 15 story building seated upon a bosun's chair while anchored to a TRACTEL davit boom, did not secure the boom rotation lock per TRACTEL user manual instructions. The employee did not use rope guards to protect the suspension rope and safety line from abrasion hazards from metal flashing on the side of Rally Hotel building. The employer did not ensure that the employee was trained in how to recognize fall hazards and the procedures to be followed to minimize those hazards. This condition exposed the employee to an approximate fifteen story fall hazard.

29 CFR 1910.27(b)(2)(viii): The employer did not ensure that prompt rescue of each employee that is provided in the event of a fall: (a) Maes Cleaning and Building Services LLC at 1600 20th Street, Denver, CO 80204: On March 12, 2021, the employer did not ensure that prompt rescue was provided. At approximately 2:20 pm, the suspension line attached to bosun's chair snapped. The employee climbed onto an eighth floor window ledge to wait for the Denver Fire Department to rescue him. The employer did not ensure that a rescue plan was in place and that all employees were trained on a rescue plan. This condition exposed the employee to an approximate eight story fall hazard.

29 CFR 1910.27(b)(2)(ix): The employer did not ensure that the ropes of each rope descent system are effectively padded or otherwise protected, where they can contact edges of the building, anchorage, obstructions, or other surfaces, to prevent them from being cut or weakened: (a) Maes Cleaning and Building Services LLC at 1600 20th Street, Denver, CO 80204: On March 12, 2021, the employer did not ensure that the ropes of each rope descent system were effectively padded or otherwise protected. An employee washed windows of a 15 story building seated upon a bosun's chair while anchored to a TRACTEL davit boom. The ropes were not protected from metal flashing that the ropes contacted. The main suspension line was cut through and the safety line was partially cut through. This condition exposed the employee to an approximate eight story fall hazard.
