29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii). On the west side of the construction site, employees worked from a tubular welded frame scaffold that was not fully planked on all working levels and were exposed to falling hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. On the west side of the construction site, employees were observed working from the second tier tubular welded frame scaffold without a fall protective system or guardrail system while performing masonry work and expose to fall hazards of 13 feet to the ground below.
