This condition exposed the employees to the hazard of electrical shock. (b) 48forty Solutions, LLC at 8221 East 96th Avenue, Henderson, CO, 80640: On or before March 18, 2021, the electrical control box located in the kiln was left uncovered, thus exposing energized parts. This condition exposed the employees to the hazard of electrical shock.

The employer used an extension cord instead of fixed wiring near the north west pallet construction stations to power the security cameras installed on the ceiling in between the pallet construction stations. This condition potentially exposed the employees to electrical shock hazards.
