29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a- 61 North Wood Drive, Franklin Lakes NJ (7 on 12 pitched roof): Employees engaged in roofing activities using pneumatic nail guns were exposed to eye loss/eye injuries due to struck by hazards as eye protection had not been utilized. Condition noted on or about 03/19/2021

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a- 61 North Wood Drive, Franklin Lakes NJ (7 on 12 pitched roof): Employees engaged in roofing activities on a 7 on 12 pitched roof were exposed to fall hazards of approximately 25ft to the concrete below as a means of fall protection had not been utilized. Condition noted on or about 03/19/2021  Extreme Roofing and Siding LLC was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13): which was contained in OSHA Inspection Number 1508878, Citation # 1, Item # 2, issued on 02/01/2021 with respect to a work place located at Lot-21 E Crescent & Roxbury Road, Mahwah NJ 07430. This citation became a final order on 03/29/21
