29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  Dancing Eagle Way, San Antonio, TX:  On or about March 19, 2021 and at times thereto, workers were exposed to struck by hazards while using hand tools to cut ventilation piping, without adequate eye protection.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  1714 Dancing Eagle, San Antonio, TX. 78258: On or about March 19, 2021, and at times prior thereto, workers were exposed to fall hazards while descending extension ladders whose side rails did not extend 3 feet above the upper landing.
