29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about March 3, 2021, employees were exposed to a struck by hazard while performing siding activities on a new residential home located at 1072 Skyland Falls, Hinckley, Ohio 44233.  Employees were working on upper areas of the home with tools and materials that could have fallen on lower employees and the employer did not ensure that the lower employees were wearing protective helmets.

Employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about March 3, 2021, employees were exposed to a fall of approximately 12 feet while performing siding activities on a new residential home located at 1072 Skyland Falls, Hinckley, Ohio 44233.  Employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.

An employee was using an A Frame ladder on the roof area and the employer did not ensure that they were using it for the purpose it was designed.
